technology. In addition to more than 350 other publications, he has coauthored
more than 30 books including Antidepressant Treatment, Depression
and its Treatment, Anxiety and its Treatment, Primer of
Lithium Therapy, Lithium Encyclopedia for Clinical Practice,
Fearless Flying, Run to Reality, and patient booklets published
by Madison Institute of Medicine including Obsessive Compulsive Disorder,
Panic Disorder and Agoraphobia, Social Anxiety Disorder,
and Fearful Flyer's Guide.
Dr. Greist received
his B.A. from Princeton University in 1961 and his MD from Indiana University
in 1965. Following a medical internship at the University of Wisconsin,
he completed residency training in internal medicine at the University
of Wisconsin where he was chief resident in 1969-1970. In his final year
of psychiatry residency at the University of Wisconsin (1970-1971), he
was a fellow in child psychiatry; he joined the faculty of the department
in 1971. Dr. Greist is board certified in psychiatry.

Dr. Jefferson received
his MD from the University of Wisconsin. Following a medical internship
at St. Luke's Hospital in New York, he completed a residency in Internal
Medicine at the University of Wisconsin and a Cardiology Fellowship at
the University of Chicago. After serving with the military as a research
cardiologist, he completed a residency in Psychiatry at the University
of Wisconsin. He joined the faculty in 1974, was tenured in 1978, served
as Professor of Psychiatry from 1981 -1992 and as Director of the Center
for Affective Disorders from 1983 - 1992. He is board certified in both
Psychiatry and Internal Medicine.
Between 1992 and
1998, Dr. Jefferson served as a Distinguished Senior Scientist at the
Dean Foundation for Health, Research and Education. In 1998 Dr. Jefferson
became a Distinguished Senior Scientist at the Madison Institute of Medicine.
He is also Clinical Professor of Psychiatry at the University of Wisconsin
Medical School. In addition, he is Cofounder of the Lithium
Information Center, the Obsessive Compulsive
Information Center, and the Bipolar
Disorders Treatment Information Center and a Director at Healthcare
Technology Systems. His major clinical and research interests are in mood
and anxiety disorders, psychopharmacology and the medical/psychiatry interface.
In addition to numerous articles in professional journals and chapters
in books, he has co-authored books including Handbook of Medical Psychiatry,
Depression and Its Treatment, Anxiety and Its Treatment, Primer of Lithium
Therapy, Lithium Encyclopedia for Clinical Practice, and Neuropsychiatric
Features of Medical Disorders and patient booklets in the Madison
Institute of Medicine's guide series, including Lithium and Manic Depression,
Divalproex and Manic Depression, Depression and Antidepressants, and
Antipsychotic Medications. Dr. Jefferson has lectured extensively
throughout the world on these and related topics.

David J. Katzelnick,
MD is a Director at Healthcare Technology Systems in Madison, Wisconsin and Clinical Associate Professor
of Psychiatry at the University of Wisconsin Medical School, Madison.
His major clinical and research interests are in mood and anxiety disorders,
psychopharmacology, diagnosis of mental disorders in primary care and
medication education. He has lectured extensively on mood and anxiety
disorders and ways to improve treatment of mental disorders in real world
settings. He has been principal investigator for a number of retrospective
and prospective randomized trials of treatment of mental disorders in
managed care. He has written numerous articles and is coauthor of
the Madison Institute of Medicine patient booklets including Social
Anxiety Disorder and Posttraumatic Stress Disorder.
Dr. Katzelnick received
his MD from the University of Colorado. He completed an internship in
internal medicine at St. Joseph's Hospital in Denver and a residency in
psychiatry at Massachusetts General Hospital in Boston. From 1990 to 1992
he was assistant professor of psychiatry at the University of Wisconsin
Medical School, where he directed the Center for Affective Disorders inpatient
unit. He is board certified in psychiatry.
